COPAY ADVANCED OR PREVENTIVE CARE WITH MEDICAL SHARE PLAN:
Choose your Base Plan - the Copay Advanced Plan or the Preventive Care plan. Then click on Medical Share plan with the $1,000 IUA option. You must enroll in both parts of the plan.*
MEDICAL SHARE PLAN ONLY:
You can enroll in the Medical Share Plan only with a $1,000 IUA. Note, this plan does not include doctor, prescription, urgent care co pays or cover preventive care. This plan covers expenses for an illness or an injury over $1,000*
*Conditions treated in the previous (24) months will not covered for one year. See member guidelines for details.
DENTAL AND VISION PLANS:
Only those who choose the Copay Advanced plan or the Preventive Care plan can opt in for dental and vision.
Open Enrollment:
The open enrollment period will begin Tuesday, September 1 and run through Monday, September 7 at noon (CST).
Employer Contribution:
$75 per month will be contributed to the cost of healthcare for employees who meet qualifications (30 hours per week).
